The classic case of "displaced in time and space" was not a factor slowing, if not hindering, timely investigation of the occurrences. Statistics indicate that organizational systemic errors within aircraft maintenance organizations are not limited to one organization or one region. In the three accidents analysed here, the behaviour of the organizations and the individuals within the organizations before the occurrences was similar. For example: Maintenance and inspection personal failed to adhere to established methods and procedures ( active failure ) . . Those responsible for ensuring adherence to established procedures and methods failed to supervise not in 'one-offs' but in what were symptomatic of longer-term failures (active and latent failures); . . High-level maintenance management failed to take positive action to require compliance with procedures as prescribed by their respective organizations (latent failures); . . Maintenance work was performed by personnel who were not assigned to do the job but who, with good intentions, started the work on their own initiative (active failure fostered by the two previous latent failures); and . Lack of proper and/or positive communication was evident, extending the chain of error which led to the accidents (latent failure). . . . . As indicated in 1.11, one of the basic elements of the aviation system is the decision maker (high-level management, companies' corporate or regulatory bodies) who is responsible for setting goals and for managing available resources to achieve and balance aviation's two distinct goals: safety and on-time and cost-effective transportation of passengers and cargo. When viewed through both the reason and the SHEL models, it is not difficult to see why and where errors were committed. . ............................................ . . . HUMAN ERROR . . . . 1.15 Human error rather than technical has the greatest potential to adversely affect contemporary aviation safety. the Boeing commercial airplens company recently analysed 220 documented accidents and found the top three causal factors to be . . . 1. flight crews not adhering to procedures (70/220) 2. maintenance and inspection errors (34/220) 3. design defects (33/220) . . the following quotation illustrates this point: . . "Because civil aircraft are designed to fly safaly for unlimited time provided defects are detected and repaired,safety becomes a matter of detection and repair rather then one of aircraft structure failure. in advance, located positively before they become dangerous, and eliminated by effecive repair, in one sense , theen we have changed the safety system from one of physical defects in aircraft into one of errors in complex human-centred systems. . . . The increasing significance of human error is not aircraft engineering. hollnagel conducted asurvey of the human factors literature to identify the extent of the human error probiem . in the 1960s, when the problem first begen to attract serious attention . the estimated contribution of human error to accidents was around 20% hn the 1990s. this figure has increased fourfold to 80%. there many possible reasons for this dramatic increasa, but thera are three which relate to aircraft engineering . . . . the reliability of mechanical and electronic components has increased markedly over the pst thiry years. people have stayed same. . . . Aircraft have become more automated and more complex . The current generation of Boeing 747 – 400s and Airbus A340s has duplicated or triplicated flight management system . This may have reduced the burden on the flight crew but it has placed a greater demand on aircraft maintenance technicians, many of whom acquired their basic training in mechanical rather than computerized control system . This suggests a mismatch of the Liveware –Hardware (L-H) and Liveware – Software (L-S) components of the SHEL model . . . . Increase aviation system complexity creates the potential for organization accidents in which latent procedural and technical failures combine with operational personnel errors and violations to penetrate or circumvent defences as the Reason model suggests . . . . . تشير الاحصائات بأن الأخطاء لم تكن ناتجة أو مسببة من قبل منظومة (شركة واحدهـ) بعينها أو لم تحدث في منطقة معينه دون سواها فالأخطاء يمكن تقع من أي منظومة و في أي مكان فعلى سبيل المثال عند تحليل الثلاثة حوادث التي بين يدينا نجد ان سلوك كل منظمة مشاة لسوك المنظومة الأخرى قبيل وقوع الحادث. . . . فعلى سبيل المثال: أفراد الصيانة و التفتيش أخفقوا بالالتزام بالأسس الضرورية إتباع الطرقِ والإجراءات السليمة و المطلوبة منهم. (هذا الإخفاق مازال مستمر) . . . . وعلى المسئولين عن ضمان الالتزام بالإجراءات و الطرق الأساسية التمسك بتلك الإجراءات لضمان عدم الإخفاق فقط ولكن تلافياً لحدوث ذلك الإخفاق و الذي من الممكن أن يسبب حالة فشل طويل الأجل (هذا الإخفاق مازال مستمر) |

In short , complexity acts to shift the errors to other people . . . إن الإدارة العليا للصيانة فشلت في إيجاد الإجراءات الإيجابية و الفعالة لإرغام الجميع باعتماد الإجراءات المنصوص عليها ضمن تعليمات منظومتهم. (هذا الإخفاق مازال مستمر) كانت أعمال الصيانة توكل لأشخاص غير معنيين أو غير مؤهلين لأداء تلك الوظائف المناطة بهم، وكانوا يؤدون تلك الأعمال بما حصلوا عليه من معلومات (مازال الإخفاق وارد في ذلك طبقاً لما لإخفاقي السابقين) الفشل أو القصور بالاتصال الإيجابي السليم كان واضح في سلسلة الإخفاقات التي تتسبب في وقوع الحوادث (خطأ كامن) وكما هو مبين في الدليل رقم 11-1 أن صنع القرار يعتبر من العناصر الأساسية لمنظومات الطيران وهي (الإدارات عالية المستوى الخاصة بالطيران ، الشركات التي تعمل في مجال الطيران أو الهيئات الرقابية) ونتساءل من هو المسئول عن تحديد الأهداف وإدارة الموارد المتاحة من اجل تحقيق التوازن في الطيران بين نقل الركاب و نقل البضائع. وعندما نتابع مثل تلك الإحداث يكون واضحاً معرفة متى و أين حدث ذلك الخلل. الأخطاء البشرية: تمثل الأخطأ البشري عامل أكبر بكثير من الأخطاء التقنية والتي لها الأثر السلبي على سلامة الطيران المعاصر. اذلك قامت شركة الطائرات التجارية (بوينج) بتحليل 220 وثيقة متوفرة لديها و وجدت أن لمراكز الثلاثة الأولى و المسببة لحويدثات تؤثر سلباً على سلامة الطائرات هي:
الأسئلة التالية توضح النقطة : بما أن تصاميم الطائرات المدنية يعتمد على تصاميم ذات آمان عالي لكي تطير لفترات طويلة و غير محددة فإنه ربما يتم اكتشاف العيوب و إصلاحها، وبذلك تكون السلامة عامل للكشف عن العيوب والأخطاء و أصلاحها بدلا من أن يبقى ذلك العيب مؤثراً على هيكل إحدى الطائرات. اكتشاف العيب مسبقاً يعد أمراً إيجابياً ومهم التعامل معه قبل أن يصبح ذلك العيب خطر ومؤثر على سلامة الطيران ، لذلك فإنه يتم تغيير أحد الأنظمة التي تؤثر على الطائرة سلامة الطائرة إلى نظام معقد يكون للأنسان دور فيه أن زيادة الاهتمام بالخطأ البشري لا يعتبر خطأ في الطائرة أو تصميمها. وقد أجرى بحث لتحديد مدى أهمية الأخطاء البشرية. ففي الستينات عندما يحدث خلل أو حادث فقد كانت النسبة المؤوية التي يعطيها الخبراء للخطأ البشري تعادل 20%. أما في التسعينات فقد بلغت النسبة التي تعطى للأخطاء البشرية 80% . يجوز أن هناك العيد من الأسباب لأتساع تلك النسبة التي تعطى للأخطاء البشرية و لكن هناك ثلاث مسببات تكون على علاقة بتصميم و هندسة الطائرات. إن الاعتماد على أنظمة الميكانيكا و الكهرباء تطور كثيراً خلال الثلاثين سنة الماضية أما الناس فإن تصرفاتهم و قراراتهم بقية كما هي عليه منذ ثلاثين سنة. أصبحت الطائرات تعتمد على أنظمة الآية معقدة جدا.فالجيل الحالي من بوينج 747-400 أو ايرباص a340 يحتوي على أنظمة مزدوجة أو ثلاثي الأنظمة من الأنظمة المتخصصة في تشغيل الطائرة (مثال نظامين كهرباء و خطين للهيدرولك و غير ذلك). وهذا أدى إلى تقليص الجهد على طاقم الطائرة لمتابعة كل جهاز أو كل نظام ولكن رفع الطلب على فنيي صيانة الطائرات. ولكنك تجد أن العديد من فنيي صيانة الطائرات تلقى تعليمة و تدريبه على أنظمة ميكانيكية تقليدية ولم يتعود على العمل في بيئة تعتمد على أنظمة حواسيب وبرمجيات، وهذا يدل على عدم توافق تأهيل هؤلاء على الأنظمة الحديثة من البرمجيات كأن يتابع الأجهزة التي على اليسار أو الأجهزة التي على اليمين و هكذا الزيادة في تعقيد أنظمة تشغيل الطائرات أدى إلى الارتفاع في نسب الحوادث الكامنة وهذا بسبب القصور في أداء الإجراءات التقنية السليمة، لذلك تجد أن الأخطاء التقنية تكون متلازمة مع الأخطاء البشرية وهذا سببه في انتهاكات الأصول السليمة للفحص أو الالتفاف على النظام و كل ذلك يؤدي إلى تعقيد المسألة و كذلك يؤدي إلى أخطاء أخرى |
