UAE Cites Chart Problems In IL-76 Close Call

Pilots of a Badr Airlines Ilyushin IL-76TD four-engine heavy cargo jet came within feet of landing on workers and equipment on a new, unfinished runway at United Arab Emirates’ (UAE) Sharjah International Airport in late June in 2014, partly because of a Jeppesen airport diagram that appeared to show the runway was open.

The daytime incident, which took place in good weather at the conclusion of a flight from Khartoum, Sudan, scattered workers on the unfinished Runway 30 that is being built to replace the in-service Runway 30, says the UAE’s Air Accident Investigation Sector (AAIS) in a final report on the incident (PDF). The pilots—seeing a metal, upright “X” lit in white lights placed near the runway end to mark it as closed—began the go-around at 0.3 nm from the threshold of the new runway at an altitude of 50 ft., low enough so that the jet blast from the go-around moved a 1-ton piece of equipment backward on the runway. There were no injuries or damage to the aircraft.

AAIS issued 11 recommendations, with five directed at Badr Airlines, in part to correct poor crew-resource management in the cockpit and inadequate pre-flight dispatch briefings on what was known to be a confusing runway-identification process due to the construction.

Air traffic controllers in Dubai cleared the aircraft for a visual approach to Runway 30 at Sharjah, then handed off the aircraft to the Sharjah tower. There was confusion among the crew as to which of two parallel runways—the existing Runway 30 or the new one—to use throughout the approach. Tower controllers watching the arrival did not realize the crew ultimately selected the closed runway.

The Jeppesen “temporary revision” chart, dated May 30, showed the unfinished Runway 30 as a black solid line and the actual runway as a faint gray line, labeled as Taxiway B. The chart also included a note cautioning pilots to “correctly identify the runway in use,” but the AAIS says the note “was not sufficient to alert the crew to the fact that the chart depicted a future configuration which would be effective at a later date.” The investigators also noted the power of graphics over text, saying the “conscious part of the brain is stimulated by bigger, wider, more visible, tangible shapes.”

AAIS says a second incident occurred less than two weeks later when another aircraft, using the same Jepessen temporary revision airport diagram, mistakenly selected the unopened new runway and was commanded by controllers to go around. Follow-on instructions called for the chart to be “removed from circulation,” the AAIS says.

Jeppesen’s Office of Aviation Safety reviewed the chart as part of the Badr investigation, concluding that “a better airport plan view . . . would have utilized the ‘Area Under Construction’ Symbolization (dashed outline) around the area where the new runway was being constructed,” leaving the current, or active runway, shown as a solid black line with a runway designation.

The AAIS issued one recommendation to Jeppesen asking the company to “carry out a suitable evaluation of likely pilot human factors aspects in the use of charts,” and use the findings for future charts.

“The decision to publish a temporary construction page is based on source evaluation, as well as input from the responsible airport authority and customers,” Jeppesen, a Boeing subsidiary, told Aviation Daily in a statement. “The recommendations made in this report are being reviewed and considered by our technical standards and human factors teams in conjunction with customers and other stakeholders.”

Among the four recommendations to the Sharjah Airport Authority, which subcontracts for air traffic services (ATS), is to review the contract and change a key performance indicator (KPI) known as the “go-around KPI.” The provision cuts ATS payments if controllers are responsible for a more than a certain number of go-arounds per month, creating a potential conflict of interest.

“The Investigation believes that the existence of the ‘go-around’ KPI in the contract between the Airport Authority and the ATS Provider is a source of hazard that may discourage controllers from reporting ‘go-arounds,’ or from describing the ‘go-around’ accurately, or to assign responsibility to the flight crew for its execution,” the AAIS says. After the Badr incident, Sharjah tower controllers reported that the aircraft’s lowest altitude during the go-around was 500 ft.
